What do we do?

Our team of skilled nurse practitioners deliver primary care services to elderly and homebound patients throughout the region. They provide both routine and urgent house calls in-person. If needed, a telehealth visit can also be arranged. Our experienced team is able to provide the same level of care you expect from a traditional primary care office, but in the comfort and convenience of your home. As a result, our patients will never have to deal with arranging costly transportation and waiting weeks to be seen by a provider.

In addition to providing the medical care you expect and deserve, our team is trained to maintain close communication with caregivers and loved ones. We typically provide same day updates through the phone or over email to family members that prefer to be updated on their family member’s care.

What are our operating hours?

Although we keep traditional business hours, we strive to work around our patients and their loved ones busy schedules to ensure everyone can be involved in managing their health. In certain emergencies our providers may even be able to visit over the weekend as well.

What’s our expertise?

Our providers are experienced with working in the unique environment of each patient’s home. They are trained with identifying and managing both chronic and acute medical conditions that are common among the elderly and homebound.

In addition, our managing provider Jessica Abrams is a certified psychiatric mental health nurse practitioner (PMHNP) who can attend to the mental health needs of our patients. She is experienced in treating common mental health problems including Alzheimer’s disease, dementia, and anxiety disorders.

Will I be assigned a single provider?

We find that most patients prefer to be seen by the same provider and as such, will assign a provider from our team to be your sole primary care provider. In times where a patient has an emergency and their assigned provider is unable to make an urgent visit, another provider from our team will conduct the visit. Your provider will receive an update of the visit and resume your care during the next scheduled visit.

Will my new provider replace my primary care physician?

Most of our patients reach out to Ikon Health when they have trouble accessing healthcare outside of their home. As a result, we traditionally assume all primary care duties and will work with your former primary care provider to ensure we receive your complete medical history. All of our patients are free to switch back to their former provider if they choose to in the future.

Will insurance cover the cost of a house call?

Ikon Health does not charge a fee for in-home care. All expenses are handled the same as with a traditional medical office. We accept patients covered by Medicare, Medicaid, and work with most supplemental insurance plans, as well as provide private pay options.
